Calling All CarersMay 2011Do you provide substantial and regular care to another person? Do you look after a relative, friend or neighbour of any age who can't manage at home without help?
Heath Lane Surgery provides lots of information for carers to assist in all sorts of situations, from respite care to help with benefits.

If you are a Carer and have not already registered as a carer at Heath Lane Surgery, please download the carers consent form, fill in your details and bring it along to the Surgery for our Carer's link worker - Ruth Allen. A carer's information pack will then be sent to you with lots of free information and advice.

You will be put on the Hinckley carer's data base (on completion of the carers consent form) who will also provide the latest up to date information of services. Heath Lane Surgery will also invite you for an annual health check.
