Elderly Persons Care Coordinator (ECPCC) - UpdateDecember 2010 As Elderly Persons Care Co-ordinator, between August and October this year, I saw a total of 127 people over 75yrs olds; 75 of these were identified as having unmet social and health needs, and were referred to various professionals or agencies for help. All of these people now have extra help and support, either financially, socially or physically. The role has benefited a very large number of people in a vulnerable age group, and the vast majority of them have a better quality of life as a direct result of the work done in the last year. I have included a few examples of feedback received from patients:
"I had no idea I could claim extra money, it's made such a difference to my life"
"I don't know how I would have managed to cope with Mum if she didn't have the carers every day. It's given me some peace of mind again"
"Getting the adaptations to the house has given me a bit more independence, I can't thank you enough" These are some typical responses from people who have received some extra help.
As the role was only a pilot for 1 year, it will be coming to an end very soon, but hopefully more information and help will continue to be available to our older population via the surgery or Social Services. May I end by saying it has been a sheer delight to meet so many wonderfully resilient elderly people, and it has been my pleasure to be EPCC at Heath Lane for the last year.
