Registration

In order to register with our practice, please check with our reception staff that you live within the practice catchment area. If this is the case, you will need to bring your NHS medical card, or if you have misplaced your card, then you can simply complete a registration form.

To complete the form you will need the following information:
• Full name, including previous names
• Date and place of birth
• Last address (including postcode)
• New address and post code
• Last name and address of your previous Doctor (this is essential)
• NHS number - if not known please contact your previous doctor

In addition to the above, you will also need to complete a new patient questionnaire, and you will be invited to attend a new patient health check.
